A problem-oriented approach to the keeping of medical records is said to enable more efficient record use, more effective communication, and improved patient care. To our knowledge, no reports describe use of the problem-oriented medical record in community hospitals. The problem-oriented system was implemented in our clinics, and a number of problem-oriented and source-oriented records chosen at random were compared. There wasno statistical differencebetween the two groups of records in the number of problems identified or in the availability of objective data and adequacy of follow-up. (JAMA229:549-551, 1974)
