Oral food challenges (OFCs) are important for the management and diagnosis of food allergies.1 OFCs can enable children to avoid the complete elimination of the causative food2,3 and may accelerate oral tolerance.4,5 Stepwise OFCs were recommended in the 2017 Japanese food allergy guidelines.6 We previously reported the usefulness and safety of stepwise2,3 and single-dose OFCs.7 Because these previous studies were retrospective single-center trials,2,7 they had an inherent bias that may have caused low positive rates of OFCs.
