Biatrial lesions cause acute mitral obstruction in a 34-year-old man with syncope and blue nevi.
Case Report (n=1)
This case highlights the clinical presentation of biatrial space-occupying lesions causing acute mitral valve obstruction in a young man with recurrent syncope and blue nevi.
May warrant echocardiography in young syncope patients with blue nevi; single case leaves biatrial mass etiology and management open.
A 34-year-old man was admitted to our hospital because of recurrent syncope without prodrome. On examination, he had a blood pressure of 98/63 mmHg, a pulse rate of 89 beats/min, and an oxygen saturation of 95%. He had many irregular blue nevi on his lips (Figure 1). The emergency physician auscultated a “plop” in the area of the mitral and tricuspid valves. Echocardiography revealed space-occupying lesions in both the left and right atria, which caused an acute obstruction of the mitral valve (Figure 2A).

Yue et al. (2023) conducted a case report in Syncope (n=1). Echocardiography in a 34-year-old man with recurrent syncope and blue nevi revealed biatrial space-occupying lesions causing acute mitral valve obstruction.
