SUMMARY A multidisciplinary assessment, including videofluoroscopy, was carried out on 14 children with feeding difficulties associated with neurological problems. Recommendations were made on all aspects of feeding and the trunk position was changed for half of the patients. A later interview with the parents confirmed that the recommendations had been helpful. The optimum trunk position for feeding was determined during videofluoroscopy by positioning the patient in the erect or reclined position, and also by analysis of relative difficulties during the oral and pharyngeal phases of swallowing. Those with difficulties mainly in the oral phase fed best in the reclined position; those with difficulties mainly in the pharyngeal phase fed best in the erect position. particularly if they had upper‐oesophageal sphincter spasm in association with a tonic labyrinthine reflex. La position optimale du tronc pour I'alimentation fut determinee durant la fluoroscopie en plaqant le patient en position erigee ou penchee en arriere mais également par analyse des difficultes plus ou moins importantes durant les temps oraux et pharyngés de la deglutition. Ceux des patients dont les difficultés se manifestaient principalement durant le temps oral bénéficiaient d'une inclinaison en arriere pour I'alimentation; ceux dont les difficultés etaient principalernent reliees au temps pharynge etaient nourris au mieux en position erigee, tout spécialement s'ils avaient un spasme oesophagien supérieur associe a un reflexe tonique labyrinthique. ZUSAMMENFASSUNG Beurteilung von Fut ferungsproblemen bei Kindern mit neurologischen Störungen durch VidcoJluoroskopie Bei 14 Kindern mit Fütterungsprobltmen in Verbindung mit neurologischen Storungen wurde eine interdisziplinäre Untersuchung, einschlieRlich Videofluoroskopie, durchgefuhrt. Es wurden Empfehlungen für alle Aspekte des Fütterns gegeben und bei der Hälfte der Patienten wurde die Körperhaltung verändert. Ein späteres Interview mit den Eltern bestätigte, dab die Empfehlungen hilfreich gewesen sind. Die optimale Körperhaltung fur die Nahrungsaufnahme wurde durch videofluoroskopische Untersuchungen bestimmt, indem der Patient in aufrechte oder zurückgelegte Position gebracht wurde, sowie durch eine Analyse der Schwierigkeiten in der oralen und pharyngealen Phase des Schluckens. Patienten, die ihre Hauptprobleme in der oralen Phase hatten, wurden am besten in zurückgelegter Position gefuttert, und die mit Schwierigkeiten in der pharyngealen Phase wurden am besten in aufrechter Haltung gefüttert, insbesondere wenn sie Spasmen im oberen Teil des Osophagus in Verbindung mit einem tonischen Labyrinthreflex hatten. La posición óptima del tronco para la alimentación se determinó en el curso de la videofluoroscopia colocando al paciente reclinado o erguido y analizando las dificultades relativas durante 13s fases oral y faríngea de la degulticion. Aquellos pacientes que tenian dificultades expecialmente en le fase oral se alimentaban mejor en la posicion reclinada, mientras que los que tenian la dificultad especialmente en la fase faringea se alimentaban mejor en la posicion erguida, especialmente si tenian un espasmo esofágico alto asociado a un reflejo tonico laberintico.
