This article aimed to evaluate comparative outcomes of sleeve gastrectomy with antrum resection (SG-AR) versus antral preservation (SG-AP) regarding weight loss efficacy and surgical complications. A comprehensive database search up to January 2025 identified 14 comparative studies for statistical synthesis using Stata 18.0. Pooled data revealed non-significant disparity in operative time (SMD = 0.05, 95 % CI: 0.04-0.14, P = 0.577) or hospitalization duration (SMD = 0.07, 95 % CI: 0.13-0.27, P = 0.031) between two groups. SG-AR exhibited superior weight reduction within a 24-month follow-up period. Additionally, the analysis of complications indicated no statistically variations in postoperative bleeding (OR = 0.55, 95 % CI: 0.28–1.06, P = 0.558), leakage (OR = 1.53, P = 0.965), gastroesophageal reflux disease (OR = 1.29, 95 % CI: 0.68–2.42, P = 0.620), or weight regain (OR = 0.34, P = 0.047).
