Poststroke cognitive impairment (PSCI) affects up to 60% of stroke survivors within the first year, shaped by prestroke and poststroke factors. Cognition can recover or decline after stroke. Intraindividual decline may be missed in those with high baseline cognitive performance. Determinants of PSCI may not be the same as those for cognitive decline.
