To compare the viral sampling efficacy of rayon versus nylon flocked swabs, and nasopharyngeal versus oropharyngeal sites, in older adults.
Enrolled 223 newly admitted patients aged 60 or older (mean age 74.9 years, SD 9.0) at a hospital in Norway and assessed them for respiratory symptoms.
Obtained paired mucosal specimens from both the nasopharynx and oropharynx using both rayon swabs and nylon flocked swabs.
Quantified and compared recovered viral loads across swab materials and anatomical sites using real-time polymerase chain reaction (PCR).
Respiratory viruses were detected in 11% of symptomatic patients.
Nylon flocked swabs collected a 4.8-fold higher viral load than rayon swabs across both sampling sites (95% CI 1.3–17, p = 0.017).
Nasopharyngeal swabs yielded a 19-fold higher viral load than oropharyngeal swabs regardless of swab type (95% CI 5.4–67.4, p < 0.001).
