Key result
Complete revascularization is linked to lower mortality and MI versus culprit-only strategy in elderly AMI.
Why the study?
The benefit of complete coronary revascularization in older patients with STEMI or NSTEMI is uncertain.
Does complete revascularization reduce all-cause mortality, cardiovascular death and myocardial infarction in elderly patients with STEMI or NSTEMI compared to culprit-only revascularization?
Observational
Does complete revascularization reduce all-cause mortality, cardiovascular death and myocardial infarction in elderly patients with STEMI or NSTEMI compared to culprit-only revascularization?
Complete revascularization may improve clinical outcomes compared to culprit-only revascularization in elderly patients with acute myocardial infarction.
May support complete revascularization in elderly multivessel ACS; leaves open confirmation in dedicated RCTs.
Key Points The benefit of complete coronary revascularization in stable patients with STEMI and multivessel disease is well established; however, the extension of this benefit in older patients or with NSTEMI is uncertain. This observational study including elderly patients presenting with STEMI or NSTEMI showed that a complete as compared with a culprit‐only revascularization may reduce the risk of all‐cause mortality, cardiovascular death and myocardial infarction. Additional evidence from randomized controlled trials is needed to confirm these findings

Spirito et al. (2022) conducted an observational in Acute myocardial infarction (STEMI or NSTEMI). Complete revascularization vs. Culprit-only revascularization was evaluated on All-cause mortality, cardiovascular death and myocardial infarction. Complete revascularization compared with culprit-only revascularization may reduce the risk of all-cause mortality, cardiovascular death, and myocardial infarction in elderly patients with AMI.
