Patients presenting with apparently idiopathic pulmonary embolism should undergo early echocardiography to exclude rare but treatable right heart sources such as atrial tumors.

A 69-year-old man with recurrent pulmonary embolism died six months after presentation and was found at necropsy to have a 50 mm right atrial myxoid tumour.
