Objective To provide pathological references for endoscopic mucosal resection (EMR) in patients with early gastric cancer (EGC). Methods Pathological data of 250 EGC cases who underwent surgery in our hospital from 2005 to 2008 were retrospectively analyzed. Results There were 184 cases of well differentiated adenocarcinoma in 250 EGC patients, of which 26 were Tis tumors, 58 Tm, and 100 Tsm. Lymph node metastasis was detected in 11 well-differentiated Tsm cases, of which 1 case was type Ⅱb, and other 10 cases were type Ⅱc and type m. Other 66 cases were poor differentiated adenocareinoma, of which 29 were Tm tumors and 37 Tsm. Lymph node metastasis was confirmed in 10 poor differentiated cases, of which 2 Tm cases were type Ⅱb, while other 8 Tsm cases were type Ⅱb and Ⅱc. Conclusion Tis and well differentiated Tm are absolute indications for EMR, because there is no risk of lymph node metastasis. Well differentiated Tsm of type Ⅰ , Ⅱa, Ⅰb are relative indications for EMR, while well differentiated Tsm of type Ⅱc and m and poor differentiated adenocarcinoma are at high risk of lymph node metastasis.
