Key result
Novel risk scale predicts 30-day mitral reoperation mortality better than STS score, achieving ~0.90 AUC.
Why the study?
There was a need to develop a risk scale to predict adverse (fatal) outcomes during the hospital period after reoperation for structural dysfunction of mitral diepoxy-treated bioprostheses.
Does a newly developed risk scale accurately predict hospital mortality in patients undergoing reoperation for mitral bioprosthesis dysfunction compared to existing surgical risk scores?
Observational (n=147)
No
Does a newly developed risk scale accurately predict hospital mortality in patients undergoing reoperation for mitral bioprosthesis dysfunction compared to existing surgical risk scores?
A newly developed risk scale accurately predicts 30-day in-hospital mortality in patients undergoing reoperation for mitral bioprosthesis dysfunction, outperforming existing surgical risk scores.
May aid preoperative risk assessment in mitral reoperations; leaves open external validation before clinical adoption.

Барбараш et al. (2020) conducted an observational in Structural dysfunction of mitral diepoxy-treated bioprostheses requiring reoperation (n=147). Novel risk prediction scale vs. Existing risk scales (STS, SOFA, APACHE II, EuroSCORE, ACEF) was evaluated on Predictive accuracy (AUC) for 30-day in-hospital mortality (AUC 0.899). A novel risk scale accurately predicted 30-day in-hospital mortality in patients undergoing mitral bioprosthesis reoperation (AUC=0.899), outperforming the STS score (AUC=0.847).
