Duodenal pathology is an uncommon issue in our daily practice and the diagnosis of the lesion is often unclear. The authors present a clinical report in which a cystic lesion was identified and that changed its characteristics over the course of follow-up imaging exams and where surgery was performed due to the possibility of it being a non-benign lesion. Due to the absence of pathognomonic clinical or imaging findings, this case shows that definitive diagnosis of these lesions is a challenge. This case demonstrates the multiple questions that can arise in the surgical approach to this anatomical region.
