We present a case of intravenous immunoglobulin (IVIG)-associated intravascular hemolysis. A 52-year-old woman with a history of multiple myeloma was admitted for management of presumed cytomegalovirus (CMV) pneumonia. The patient had previously undergone an autologous stem cell transplantation with high-dose melphalan (200 mg/m2) followed by a tandem nonmyeloablative allogeneic transplant 3 months later with fludarabine/cyclophosphamide conditioning and tacrolimus plus low-dose methotrexate for graft-versus-host disease prophylaxis.
