A case of juvenile xanthogranuloma with Darier's sign in a 3‐year‐old boy was described. He had several brown macules on his face, neck, and trunk. When these macules were rubbed with a metal rod, wheal‐like erythema appeared in the area corresponding to them. This phenomenon is called Darier's sign in urticaria pigmentosa. However, the histological picture revealed a xanthogranulomatous reaction, and a few mast cells were seen surrounding the granulomatous area.
