In the guideline for type 2 diabetes mellitus published by the Dutch College of General Practioners, the choices regarding the place of glucagon-like peptide-1 (GLP-1) receptor agonists are mainly based on the 2021 systematic review and network meta-analysis by Palmer et al. The guideline recommends GLP-1 agonists for patients with ‘very high risk diabetes’, which includes patients with type 2 diabetes mellitus as well as systolic heart failure. In the meantime, new studies have been published involving patients with chronic heart failure. Unlike the network meta-analysis, these new studies do not appear to show any difference relative to placebo on cardiovascular endpoints for this particular group of patients. Is there still a place for GLP-1 agonists in the treatment of patients with type 2 diabetes mellitus and systolic heart failure?
