Background Pelvic fracture hemorrhage is one of the leading causes of death in the field of trauma. Currently, there are issues of fragmented knowledge and unclear research hotspots in this field, which makes the future research directions unclear. By using bibliometric research methods, the research scientifically explored the research hotspots and development trends, filling the gaps in this field. Methods We collected relevant publications from the Web of Science Core Collection and, after a screening and exclusion process, analyzed the included publications using bibliometric methods. By presenting the basic information of the publications, we predicted and visualized future development directions and research hotspots. Results This research included a total of 388 publications. Over the past 20 years, the number of publications in this field has shown an increasing trend year by year, reaching the highest number in 2025 (34 publications), and it is expected that the number of publications will continue to grow over the next 10 years. In terms of national contributions, the United States is the largest contributor (134 publications). In terms of institutional contributions, Chang Gung University is the largest contributor (18 publications). In terms of journals, the International Journal of Surgery has published the most articles in this field (37 publications). As for individuals, Fu, Chih-Yuan is the author with the most publications (16 publications). In terms of future research priorities, high-frequency keywords are mainly focused on surgery-related treatment areas. Conclusion This is the first bibliometric analysis in the field of pelvic fracture hemorrhage, which included all publications in this field in the past 20 years. Our results illuminate the contributions of countries and institutions, international cooperation, interdisciplinary relationships, and future research directions. This research will point out the future development path of pelvic fracture hemorrhage.
