Endothelial biomarkers outshine vascular imaging to identify endothelial dysfunction and ACS risk in HIV patients.
Why the study?
The study investigated endothelial function in HIV-positive patients with ACS to identify markers of endothelial dysfunction and risk of ACS.
Do non-invasive investigations and endothelial biomarkers identify endothelial dysfunction and high risk of ACS in HIV-positive patients?

Endothelial biomarkers, rather than non-invasive imaging like FMD or CIMT, may be more useful for identifying HIV-positive patients at increased risk of acute coronary syndromes.
AIM: This study investigated endothelial function in HIV-positive patients with acute coronary syndrome (ACS). Flow-mediated dilatation, pulse-wave velocity, carotid intima-media thickness and endothelial biomarkers were used to non-invasively investigate endothelial dysfunction. METHODS: Twenty HIV-positive patients with ACS (HIV+/ACS) were compared to 20 HIV-negative patients with ACS (HIV-/ACS) and 20 HIV-positive patients without ACS (HIV+/no ACS). RESULTS: = 0.033 and 0.024, respectively). CONCLUSIONS: Non-invasive investigations such as FMD, CIMT and PWV did not identify patients with HIV who were at high risk of ACS. Endothelial biomarkers may be more useful markers to identify HIV-positive patients who have endothelial dysfunction and increased risk of ACS.

Vachiat et al. (2020) conducted an observational in HIV and acute coronary syndrome (n=60). HIV infection vs. HIV-negative patients with ACS and HIV-positive patients without ACS was evaluated on Endothelial dysfunction (FMD, PWV, CIMT, and endothelial biomarkers) (p=0.033 and 0.024). Endothelial biomarkers, rather than FMD, CIMT, and PWV, may be more useful to identify HIV-positive patients with endothelial dysfunction and increased ACS risk (P=0.033 and 0.024).
