Multidisciplinary surgical and medical treatment yields uneventful recovery in pediatric device-related endocarditis.
Why the study?
Device-related infective endocarditis in patients with Down syndrome and congenital heart disease presents complex management challenges that require multidisciplinary approaches.
Case Report (n=1)
Highlights the complex, multidisciplinary management of device-related endocarditis in a pediatric patient with Down syndrome and congenital heart disease.
Case of transvenous lead endocarditis post-AVSD repair warrants heightened suspicion in febrile device patients; leaves open optimal pediatric lead extraction strategies.
An 11-year-old with Down syndrome, autism, prior atrioventricular septal defect repair, and a transvenous pacemaker presented with prolonged fever and raised inflammatory markers. Eight years prior, she had a group A streptococcal sepsis with infection of an epicardial pacemaker. Echocardiography demonstrated a large vegetation on the right atrioventricular valve, with encasement of the pacemaker lead. Following treatment with intravenous antibiotics, she underwent a right atrioventricular valve repair, vegetation excision, and epicardial pacemaker placement. Post-operative recovering was uneventful. This case highlights the complex, multidisciplinary management of device-related endocarditis in Down syndrome and in CHD.

Coughlan et al. (2026) conducted a case report in Infective endocarditis of the right atrioventricular valve and transvenous pacing lead (n=1). Intravenous antibiotics, right atrioventricular valve repair, vegetation excision, and epicardial pacemaker placement was evaluated on Post-operative recovery. Multidisciplinary management including intravenous antibiotics, valve repair, vegetation excision, and epicardial pacemaker placement led to an uneventful recovery in an 11-year-old with device-related endocarditis.
