Highlights the critical risk of life-threatening hemorrhagic cardiac tamponade due to drug-disease interactions between amiodarone-induced severe hypothyroidism and direct oral anticoagulants.
Hemorrhagic tamponade in severe hypothyroidism is an uncommon presentation that warrants evaluation for superimposed pathology. We present the case of a 57-year-old man with nonischemic cardiomyopathy, chronic amiodarone therapy, and atrial fibrillation on apixaban who developed advanced myxedema, complicated by a large hemorrhagic pericardial effusion and cardiac tamponade. This case highlights a rare but life-threatening convergence of amiodarone-induced severe hypothyroidism and potentiated anticoagulation, culminating in hemorrhagic tamponade.Recognition of drug-disease interactions is critical in anticoagulated patients with endocrine dysfunction who present with rapidly recurrent pericardial effusion.

Torres-Mercado et al. (2026) conducted a case report in Hemorrhagic cardiac tamponade, severe hypothyroidism, nonischemic cardiomyopathy (n=1). Concomitant amiodarone and apixaban was evaluated on Development of hemorrhagic cardiac tamponade. The concurrent exposure to amiodarone and apixaban precipitated a life-threatening drug-disease interaction culminating in severe hypothyroidism and recurrent hemorrhagic cardiac tamponade.
