BATMAN-assisted TMVR with planned IABP support successfully treats mitral regurgitation without causing LVOT obstruction.
Why the study?
Left ventricular outflow tract obstruction is a feared complication of TMVR, and strategies to mitigate this risk in patients with severe pulmonary hypertension are needed.
Does BATMAN-assisted valve-in-valve TMVR with planned IABP support prevent LVOT obstruction and maintain hemodynamic stability in a patient with severe pulmonary hypertension?

Does BATMAN-assisted valve-in-valve TMVR with planned IABP support prevent LVOT obstruction and maintain hemodynamic stability in a patient with severe pulmonary hypertension?
Planned IABP support during BATMAN-assisted TMVR may safely buffer the abrupt loading transition and prevent acute RV decompensation in high-risk patients with severe pulmonary hypertension.
Supports feasibility of BATMAN-assisted TMVR with IABP in severe pulmonary hypertension; leaves open need for prospective validation before broader use.
BACKGROUND: Left ventricular outflow tract (LVOT) obstruction is a feared complication of transcatheter mitral valve replacement (TMVR), and balloon-assisted translocation of the mitral anterior leaflet (BATMAN) has emerged as a mitigation strategy. Patients with severe pulmonary hypertension are at risk, as abrupt elimination of mitral regurgitation (MR) can destabilize hemodynamics. CASE SUMMARY: guided anterior leaflet modification. MR was eliminated with a mean transmitral gradient of 1 mm Hg, no LVOT obstruction, and stable periprocedural hemodynamics. DISCUSSION: BATMAN enabled successful TMVR despite high-risk anatomy. Prophylactic IABP facilitated hemodynamic stability during the transition from chronic MR physiology. This is among the first reports combining planned mechanical circulatory support with BATMAN-assisted TMVR in severe pulmonary hypertension. TAKE-HOME MESSAGE: BATMAN-assisted valve-in-valve TMVR with planned IABP support may facilitate safe treatment of selected patients with severe pulmonary hypertension and predicted LVOT obstruction.

Saeed et al. (2026) conducted a case report in Severe pulmonary hypertension, degenerated bioprosthetic mitral valve, and predicted LVOT obstruction (n=1). BATMAN-assisted valve-in-valve transcatheter mitral valve replacement (TMVR) with planned prophylactic IABP support was evaluated on Procedural success (elimination of mitral regurgitation, absence of LVOT obstruction, and stable periprocedural hemodynamics). BATMAN-assisted valve-in-valve TMVR with planned IABP support successfully eliminated mitral regurgitation with no LVOT obstruction and maintained stable periprocedural hemodynamics in a patient with severe pulmonary hypertension.
