Bilateral brachial plexus block is approached cautiously because of concerns regarding local anesthetic systemic toxicity, respiratory compromise, and bilateral upper-limb motor block. We describe a 60-year-old man with bilateral posterolateral elbow dislocation after a road traffic accident who underwent closed reduction and cast application under bilateral ultrasound-guided costoclavicular brachial plexus block using 10 mL of local anesthetic on each side. The procedure was completed without general anesthesia or deep sedation. This case highlights that low-volume bilateral costoclavicular block may provide effective procedural anesthesia for selected emergency bilateral upper-limb trauma when meticulous dosing, ultrasound guidance, and monitoring are used.
