Background: Miliaria rubra (prickly heat) is an environmentally induced inflammatory skin condition triggered by eccrine sweat duct occlusion, skin hyper hydration, and secondary bacterial colonization under hot, humid ambient conditions. Patients experience intense prickling, burning, and pruritus that disrupt daily comfort, sleep, and epidermal barrier integrity. Conventional talcum powders provide superficial tactile glide but do not address sweat duct occlusion, microbial proliferation, or neurosensory itch pathways. Objective: To elucidate the neurosensory, biophysical, and antimicrobial mechanisms of Tetmosol® Prickly Heat Powder, an Ayurvedic multi-target therapeutic formulation for prickly heat containing Zea mays starch (12% w/w), standardized Mentha arvensis extract (Pudina Ark, 1.75% w/w), calcined mineral zinc (Jasad Bhasma, 10% w/w), and Azadirachta indica seed oil (Neem oil, 0.3% w/w) in a purified talc base. Methods: Rapid contact-kill kinetics were evaluated under ASTM E2315-16 standard time-kill methodology against major cutaneous pathogens (Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Candida albicans, Aspergillus niger) and primary axillary odour producing bacteria (Staphylococcus hominis, Corynebacterium striatum) across intervals from 1 minute to 12 hours. Clinical cutaneous cooling kinetics were evaluated in a 6-day, single-center trial involving 53 healthy human subjects (34 males, 19 females; mean age 28 years) using calibrated infrared thermography across vulnerable anatomical regions (underarms, chest, back) compared against a Clotrimazole dusting powder reference. Results: In ASTM E2315-16 assays, the formulation demonstrated rapid antimicrobial action within 2 minutes: 99.76% kill against E. coli (3-log10 reduction), 99.93% against S. aureus (3- log10 reduction), 99.85% against P. aeruginosa (3-log10 reduction), and 99.99% against C. albicans (4-log10 reduction). Against axillary malodour-generating bacteria, the powder achieved a 99.91% kill of C. striatum (3-log10 reduction) and 98.88% kill of S. hominis (2-log10 reduction) at 2 minutes, maintaining complete suppression (100% kill, 9-log10 reduction) of C. striatum and 99.94% kill (3-log10 reduction) of S. hominis through 12 hours. Clinical thermography confirmed an immediate surface cooling onset within 1 minute (approximate 1.0°C reduction), achieving a sustained mean body surface temperature reduction of 5.39°C within 15 minutes (dropping from an untreated baseline of 32.29°C down to 26.90°C), significantly outperforming the conventional Clotrimazole dusting powder reference (0.63°C reduction; mean difference of 5.44°C; p < 0.001). Conclusion: Tetmosol® Prickly Heat Powder operates via a six-in-one therapeutic paradigm: rapid two-minute broad-spectrum germ kill, rapid TRPM8 receptor-mediated sensory cooling, biophysical evaporative heat dissipation, fluid-locking perspiration absorption, and extended 12- hour malodour enzyme pathway suppression. This provides an evidence-based dermatological intervention for prickly heat and everyday tropical skin hygiene.
