Lichen planus (LP) is a benign chronic inflammatory disorder of the skin and mucosa. Malignant transformation in LP lesions is uncommon and has been primarily reported in oral LP. We report a case of a 55-year-old lady who presented with a long-standing hyperkeratotic lesion, with recent development of ulceration. Histopathology showed evidence of squamous cell carcinoma in the background of hypertrophic LP. Although cutaneous LP is not considered a pre-malignant condition, certain factors such as the hypertrophic variant and chronicity may be associated with a risk of malignant degeneration. Therefore, regular follow-up and a high index of suspicion are of utmost importance in long-standing hypertrophic LP.
