A 17-year-old presented to our clinic with symptoms of severe opioid withdrawal, including tachycardia, sweating, diarrhea, and anxiety, after abruptly stopping 7-hydroxymitragynine (7-OH). He began using 7-OH purchased from a vape shop socially 1 month prior, but his use escalated quickly, and he made two failed attempts to stop before seeking care. Withdrawal symptoms were successfully treated with a short course of sublingual buprenorphine (daily dosing 2–8 mg), and he was referred to a treatment program for ongoing mental health care.
