Key result
Beta-blocker combination therapy improves BP target achievement by ~11 percentage points over aliskiren in refractory hypertension.
Why the study?
The effectiveness of combined antihypertensive treatment including direct renin inhibitor in salt-sensitive and salt-resistant patients with refractory hypertension was unclear.
RCT (n=192)
May support beta-blocker addition in salt-sensitive refractory hypertension; leaves open confirmation versus aliskiren in randomized trials.

Skibitskiy et al. (2015) conducted an RCT in Refractory arterial hypertension (n=192). Combined antihypertensive treatment with aliskiren vs. Combined antihypertensive treatment with a beta-blocker was evaluated on Achievement of target blood pressure in salt-sensitive patients. In salt-sensitive patients with refractory hypertension, combined antihypertensive treatment including a beta-blocker achieved target blood pressure in 54.3% compared to 43.3% with aliskiren.
