Key result
Right pulmonary artery ablation successfully eliminates challenging left atrial posterior roof epicardial arrhythmia.
Why the study?
Endocardial ablation of atrial tachycardia can be challenging due to limitations in ablation energy delivery and myocardial wall thickness.
Does radiofrequency catheter ablation via a right pulmonary artery approach successfully eliminate atrial tachycardia of epicardial origin near the left atrial roof?

Does radiofrequency catheter ablation via a right pulmonary artery approach successfully eliminate atrial tachycardia of epicardial origin near the left atrial roof?
A right pulmonary artery approach may serve as an effective alternative strategy for ablating challenging epicardial atrial arrhythmias, specifically those originating near the left atrial posterior roof.
May offer an alternative access route for select epicardial atrial tachycardias; hypothesis-generating and requires prospective validation.
Introduction Due to limitations in ablation energy delivery and myocardial wall thickness, endocardial ablation of atrial tachycardia can sometimes be challenging. Methods and Results In this case, ablation of the left atrial posterior wall shifted the earliest endocardial breakthrough site from the posterior wall to the roof, suggesting a potential epicardial origin near left atrial roof. The atrial arrhythmia was successfully eliminated via a right pulmonary artery approach. Conclusion The right pulmonary artery approach may be an effective strategy for ablating epicardial atrial arrhythmias, particularly those originating from the left atrial posterior roof.

Zhang et al. (2026) conducted a case report in Atrial tachycardia (n=1). Right pulmonary artery approach for radiofrequency catheter ablation was evaluated on Elimination of atrial arrhythmia. The right pulmonary artery approach successfully eliminated an epicardial atrial arrhythmia originating from the left atrial posterior roof after endocardial ablation proved challenging.
