Antisynthetase syndrome (anti–Jo-1 positive) with inflammatory myopathy and interstitial lung disease (ILD) poses substantial challenges in pregnancy, particularly in patients with severe, treatment-refractory disease[1]. We report the first case of obinutuzumab administered during early pregnancy in a high-risk twin gestation, resulting in sustained maternal disease control and reassuring short-term neonatal outcomes.
