Multimodality imaging guides safe, precise pacemaker implantation in complex congenital heart block.
Why the study?
Pacemaker implantation in adult congenital heart disease with abnormal septal anatomy requires preprocedural planning to improve procedural safety and lead positioning.
Does multimodality imaging improve the safety and precision of pacemaker implantation in adult patients with congenital heart disease and abnormal septal anatomy?

Does multimodality imaging improve the safety and precision of pacemaker implantation in adult patients with congenital heart disease and abnormal septal anatomy?
Multimodality imaging is a valuable tool for preprocedural planning to facilitate safer and more precise pacemaker implantation in adult patients with complex congenital heart disease.
Pacemaker implantation in adult congenital heart disease with atrioventricular block warrants preprocedural planning; leaves open optimal multimodality imaging protocols in complex septal anatomy.
CASE SUMMARY: A 57-year-old man with a history of congenital ventricular septal defect and bicuspid aortic valve with surgical repair of aortic coarctation presented with dyspnea and found to be in complete heart block with junctional escape rhythm necessitating pacemaker implantation. DISCUSSION: Pacemaker implantation in adult congenital heart disease and atrioventricular block requires preprocedural planning. Multimodality imaging is important in patients with complex septal anatomy, as it can reduce procedural difficulty and improve procedural safety by identifying optimal lead positioning strategies. TAKE-HOME MESSAGE: Multimodality imaging can facilitate safer and more precise pacemaker implantation in adult patients with congenital heart disease with abnormal septal anatomy.

Singh et al. (2026) conducted a case report in Complete heart block in adult congenital heart disease (n=1). Multimodality imaging-guided pacemaker implantation was evaluated. Multimodality imaging successfully guided safe and precise pacemaker implantation in a 57-year-old man with complete heart block and complex congenital septal anatomy.
