Cardiac MRI reveals hypertrophic cardiomyopathy mimicking cardiac lymphoma on 18F-FDG PET/CT.
Why the study?
Hypertrophic cardiomyopathy can mimic cardiac involvement of lymphoma on 18F-FDG PET/CT, complicating diagnosis and management.
Case Report (n=1)
Hypertrophic cardiomyopathy can mimic cardiac lymphomatous involvement on 18F-FDG PET/CT, highlighting the importance of multimodality imaging such as cardiac MRI for accurate diagnosis.
Focal apical 18F-FDG uptake may represent hypertrophic cardiomyopathy rather than lymphoma; this single case leaves open PET specificity for cardiac masses.
18 F‑FDG PET/CT was performed on a 78‑year‑old woman for initial staging of newly biopsy‑proven follicular lymphoma. The scan revealed focal uptake at the left ventricular apex, along with multiple hypermetabolic lymph nodes in the cervicothoracic region. Subsequent cardiac magnetic resonance imaging, performed to exclude cardiac lymphomatous involvement, demonstrated asymmetric hypertrophy of the interventricular septum and apical myocardium, with late gadolinium enhancement in the apex, supporting the diagnosis of hypertrophic cardiomyopathy. Follow‑up 18 F‑FDG PET/CT after chemotherapy showed persistent apical uptake, despite metabolic remission of the lymphoma.

Chen et al. (2026) conducted a case report in Follicular lymphoma and hypertrophic cardiomyopathy (n=1). 18F-FDG PET/CT was evaluated. 18F-FDG PET/CT demonstrated focal apical uptake mimicking cardiac lymphoma in a 78-year-old woman, which was subsequently diagnosed as hypertrophic cardiomyopathy by cardiac MRI.
