Wire control and surgical bailout successfully manage rare long-sheath tip separation during calcified transfemoral TAVR.
Why the study?
Long introducer sheath tip separation during noncontrast transfemoral TAVR is a rare complication with limited technical guidance for bailout strategies.
Case Report (n=1)
Shows successful surgical bailout of long-sheath tip separation during noncontrast transfemoral TAVR, highlighting the necessity of preserving wire control and halting valve deployment.

Isomura et al. (2026) conducted a case report in Severe aortic stenosis (n=1). Transfemoral transcatheter aortic valve replacement (TAVR) and surgical bailout for sheath separation was evaluated. During noncontrast transfemoral TAVR in a patient with severe iliofemoral calcification, unexpected long-sheath tip separation occurred and was successfully managed with wire control and surgical bailout.
