Calcium supplementation relieves angina and completely normalizes ECG abnormalities in hypoparathyroidism.
Why the study?
Electrocardiographic manifestations secondary to hypoparathyroidism that mimic acute coronary syndrome are uncommon and require improved diagnostic accuracy and therapeutic management.
Does calcium supplementation resolve acute coronary syndrome-mimicking symptoms and ECG abnormalities in a patient with hypoparathyroidism?
Case Report (n=1)

Does calcium supplementation resolve acute coronary syndrome-mimicking symptoms and ECG abnormalities in a patient with hypoparathyroidism?
Hypoparathyroidism-induced severe hypocalcemia can closely mimic acute coronary syndrome with angina-like symptoms and dynamic ECG changes, which are fully reversible with targeted calcium supplementation.
May inform ECG evaluation in hypoparathyroidism; leaves open whether calcium repletion benefits extend beyond this case.
Objective: To promptly identify electrocardiographic (ECG) manifestations, including deep T-wave inversions and QT-interval prolongation, secondary to hypoparathyroidism and to improve its diagnostic accuracy and therapeutic management. Methods: We conducted a comprehensive analysis of the integrated treatment for a patient presenting with chest pain and evolving ECG changes secondary to hypoparathyroidism, supplemented by a review of the relevant literature. Results: In this case, a patient diagnosed with hypoparathyroidism presented with chest pain and dynamic ECG alterations. Following a confirmed diagnosis, treatment with calcium supplementation successfully alleviated the patient's chest pain and resulted in the complete normalization of the previously abnormal ECG findings. Conclusion: Clinical presentations mimicking angina pectoris, accompanied by secondary ECG changes such as deep T-wave inversions and QT-interval prolongation, are relatively uncommon in patients with hypoparathyroidism. A thorough evaluation combining clinical presentation, characteristic signs, and laboratory investigations can significantly reduce the rate of misdiagnosis.

Zhang et al. (2026) conducted a case report in Hypoparathyroidism mimicking acute coronary syndrome (n=1). Calcium supplementation was evaluated on Symptom resolution and ECG normalization. Calcium supplementation successfully alleviated angina-like chest pain and completely normalized deep T-wave inversions and QT-interval prolongation in a patient with hypoparathyroidism.
