Key result
Transcatheter edge-to-edge repair with the PASCAL Ace device achieved technical success, MR reduction, relief of LVOT obstruction, and symptomatic improvement in 100% of patients.
Why the study?
Dynamic left ventricular outflow tract obstruction due to systolic anterior motion of the mitral valve occurs across diverse anatomic substrates and requires evaluation of treatment options.
Does transcatheter edge-to-edge repair (TEER) relieve left ventricular outflow tract obstruction and improve symptoms in patients with systolic anterior motion of the mitral valve?
Case Report (n=5)
No
Does transcatheter edge-to-edge repair (TEER) relieve left ventricular outflow tract obstruction and improve symptoms in patients with systolic anterior motion of the mitral valve?
Transcatheter edge-to-edge repair appears technically feasible and effective for relieving systolic anterior motion-mediated left ventricular outflow tract obstruction and improving symptoms without causing significant mitral stenosis.
Supports early feasibility in small series; leaves open durability and generalizability for prospective trials.
Dynamic left ventricular outflow tract obstruction due to systolic anterior motion of the mitral valve can arise across diverse anatomic substrates. We report 5 consecutive patients with systolic anterior motion–mediated left ventricular outflow tract obstruction treated with the PASCAL Ace (Edwards Lifesciences) device at a single center. Technical success, mitral regurgitation reduction to ≤2+, relief of left ventricular outflow tract obstruction, and symptomatic improvement to New York Heart Association class I to II were achieved in all patients, without clinically significant mitral stenosis.

Zorman et al. (2026) conducted a case report in Systolic anterior motion-mediated left ventricular outflow tract obstruction (n=5). Transcatheter Edge-to-Edge Repair with the PASCAL Ace device was evaluated on Technical success, mitral regurgitation reduction to ≤2+, relief of left ventricular outflow tract obstruction, and symptomatic improvement to NYHA class I to II. Transcatheter edge-to-edge repair with the PASCAL Ace device achieved technical success, MR reduction, relief of LVOT obstruction, and symptomatic improvement in 100% of patients.
