Advanced atrial myopathy alters activation patterns and masks classic ECG features of Bachmann bundle pacing.
Why the study?
Bachmann's bundle pacing is a novel physiological pacing strategy to restore interatrial synchrony, but advanced atrial myopathy may alter atrial activation patterns and mask classical ECG features of BBP.
Case Report
Demonstrates combined ablation strategies for complex atrial arrhythmias following Bachmann bundle pacing, highlighting structural changes from advanced atrial myopathy.
Bachmann`s bundle pacing (BBP) has gained renewed attention as a novel physiological pacing strategy to treat patients with bradycardia and interatrial conduction delay.1 By pacing Bachmann`s bundle (BB), which serves as the main electrical connection between the right atrium (RA) and left atrium (LA), BBP aims to restore interatrial synchrony. However, in patients with advanced atrial myopathy, severe structural degeneration can alter the expected atrial activation pattern, occasionally masking the classical electrocardiographic features of BBP.

Kawaji et al. (2026) conducted a case report in Complex atrial tachyarrhythmias following Bachmann's bundle pacing. Combined pulsed field and radiofrequency ablation was evaluated. In patients with advanced atrial myopathy, severe structural degeneration can alter expected atrial activation patterns and mask classical electrocardiographic features of Bachmann's bundle pacing.
