Does the addition of 24-hour Holter ECG RR interval dynamics to clinical models improve the prediction of cognitive performance in patients with persistent atrial fibrillation?
The addition of 24-hour Holter-derived RR interval variability to standard clinical models significantly improves the prediction of early cognitive impairment, particularly executive function, in patients with persistent atrial fibrillation.

May enhance cognitive risk stratification in persistent AF; hypothesis-generating and requires prospective validation before practice change.

Saglietto et al. (2026) conducted an observational in Atrial Fibrillation (n=33). 24-hour Holter ECG RR interval dynamics vs. Standard clinical variables alone was evaluated on Prediction of cognitive performance (assessed by coefficient of determination, R²) (R² up to ~0.8). Integrating 24-hour Holter RR interval parameters with clinical variables significantly improved cognitive performance prediction in atrial fibrillation, reaching R² up to 0.8 for executive function.
