Superior sulcus or so-called Pancoast tumor presents a great challenge for surgical resection due to the close proximity to vital structures such as subclavian artery, vein and brachial plexus. The introduction of neoadjuvant chemo+immunotherapy, chemo+targeted therapy or the addition of radiotherapy allows locally advanced cases to be completely cured. They must be treated from a multidisciplinary team. Only this can afford the adequate preoperative diagnosis, neoadjuvant therapy, radical surgical resection with the following chest wall reconstruction and adjuvant therapy when indicated. Complete pathological response and clear resection lines (R0) are the aim of this complex treatment that offer an excellent prognosis. А 55-years old man with Pancoast low-differentiated squamous cell lung carcinoma underwent successfully neoadjuvant chemo+immunotherapy, en-block right upper lobectomy, chest wall resection and reconstruction. The final pathological result showed pathological complete response (pCR).
