Key result
Chagas cardiomyopathy is linked to ~42% lower risk of appropriate ICD therapy or death.
Why the study?
Does Chagas cardiomyopathy etiology alter the temporal risk of appropriate ICD therapy or death compared to non-Chagas etiologies in patients receiving an ICD for primary prevention?
Cohort (n=194)
Yes
Does Chagas cardiomyopathy etiology alter the temporal risk of appropriate ICD therapy or death compared to non-Chagas etiologies in patients receiving an ICD for primary prevention?
Hazard Ratio: 0.58 (95% CI 0.37–0.91)
Absolute Event Rate: 50% vs 58.3%
p-value: p=0.018
Patients with Chagas cardiomyopathy receiving a primary-prevention ICD experience a delayed accumulation of arrhythmic and mortality events compared to non-Chagas etiologies, suggesting that early event-free survival does not ensure a long-term low-risk trajectory.

Sérgio Dubner (2025) conducted a cohort in Primary-prevention implantable cardioverter-defibrillator recipients with dilated cardiomyopathy (n=194). Chagas cardiomyopathy vs. Non-Chagas cardiomyopathy (ischaemic and non-ischaemic dilated cardiomyopathy) was evaluated on Composite of first appropriate ICD therapy or all-cause death (HR 0.58, 95% CI 0.37-0.91, p=0.018). In primary-prevention ICD recipients, Chagas cardiomyopathy was associated with a significantly lower risk of the composite of appropriate ICD therapy or death compared to non-Chagas aetiologies (HR 0.58).
