Low or nonadherence to treatment is a widespread challenge in the management of rheumatic and musculoskeletal diseases (RMDs), significantly affecting disease outcomes, quality of life, and healthcare costs. The 2020 European Alliance of Associations for Rheumatology (EULAR) Points to Consider provide a comprehensive framework for the prevention, screening, assessment, and management of nonadherence in RMDs. However, real-world implementation of these recommendations remains limited. The challenge is no longer the lack of recommendations, but their integration into routine rheumatology care. This viewpoint discusses the rationale and evidence supporting implementation of the 2020 EULAR Points to Consider in clinical practice and highlights key priorities for the operationalisation of the recommendations into feasible and sustainable care strategies. Particular emphasis is placed on routine adherence assessment, patient-centred communication, multidisciplinary collaboration, and healthcare professional training as essential components of adherence support in RMD care.
