IMAGE: Prenatal diagnosis of a distal aortopulmonary window by fetal cardiac magnetic resonance imaging (MRI). CASE SUMMARY: A fetus referred for cardiac MRI because of suboptimal visualization of the great vessels on ultrasound. MRI demonstrated a distal aortopulmonary window with a characteristic heart-shaped appearance of the great vessels. TAKE-HOME MESSAGE: Fetal cardiac MRI may provide complementary anatomical information when echocardiographic visualization is suboptimal and may help characterize complex fetal outflow tract abnormalities.
