: A review of the current literature defining the problem and the causation factors as well as the diagnostic methods necessary to substantiate the clinical diagnosis and timing to surgical intervention. For years, popular teaching stated that the prognosis for recovery of cauda equina syndrome, which by definition must include loss of bowel and bladder function, is dismal following lumbar disk herniation if surgical intervention is not carried out within 6 hours. In recent years this view has been contradicted scientifically. Despite this, considerable controversy exists; it is certain that surgery should be done as expeditiously as possible.
