Gastrointestinal (GI) leiomyomas are rare benign smooth muscle tumors, most commonly arising in the esophagus and stomach. Their occurrence in the colon is uncommon, and synchronous involvement of multiple colonic sites is exceedingly rare. We report a case of a young male who underwent colonoscopic evaluation for GI symptoms, which revealed polypoidal lesions in the sigmoid colon and at the ileocecal valve. Both lesions were endoscopically resected. Histopathology with immunohistochemistry is essential for accurate diagnosis and to differentiate these lesions from GI stromal tumors. Recognition of this entity is important to guide appropriate management and avoid overtreatment.
