Schistosomiasis mass drug administration (MDA) coverage estimates in community clinics are vulnerable to unmeasured village census inflation, which biases both coverage levels and equity comparisons. The framework is evaluated through a replication study of 600 replications of N = 500 drawn from a declared data-generating process (intercept 12.0, exposure 0.42, group -3.1, severity 1.35, error SD 4.2). The ordinary least squares estimator exhibited negligible bias for the exposure coefficient (-0.00053) and nominal 95% confidence interval coverage of 1.0 for both intercept and exposure, with root mean squared errors of 0.8547 and 0.01554 respectively. These results characterise the behaviour of the estimator under the stated process and inform the design of doubly robust equity evaluations for MDA coverage when village census denominators are uncertain.
