Primary health care facility bypassing, in which patients travel past their nearest dispensary or health centre to seek care at a higher-level facility, undermines the efficiency and equity of district health systems. The analysis treats bypassing as a governance-contingent phenomenon rather than a simple function of distance or patient preference. Four governance domains are specified: administrative accountability, resource stewardship, information and monitoring, and inter-organisational coordination. The framework proposes that weaknesses in each domain alter the relative attractiveness of higher-level facilities by shaping the perceived quality, reliability and responsiveness of frontline care. The DHS indicator, with a mean of 91.964 per cent and a standard deviation of 12.468 across the 14 country surveys, is used to illustrate cross-national variation in the provider-based delivery of injectable services, a dimension of facility readiness that conditions bypassing decisions.
