Core muscle injury is an increasingly recognized condition that causes groin pain, most commonly in young, active adults. Multiple surgical interventions have been described, including rectus abdominis repair and/or adductor longus lengthening, generally reserved for recalcitrant cases. This article presents a step‐by‐step guide to an open adductor longus repair and lengthening. By performing an anatomic tendon repair combined with controlled lengthening at the musculotendinous junction, this approach aims to minimize recurrence while providing sustained pain relief.
