D-α-Tocopheryl polyethylene glycol 1000 succinate, referred to as Tocofersolan (TCFS), is an amphiphilic vitamin E derivative widely investigated as a solubilizer, micellar carrier, and functional excipient in drug delivery. Polymer and prodrug conjugation strategies may further extend the functional applications of TCFS by modulating micellization, cellular uptake, and P-glycoprotein-mediated drug efflux; however, the magnitude and direction of these effects depend on the specific polymer architecture, molecular design parameters, and experimental context. This review critically and comparatively analyzes the polymeric and prodrug conjugates based on TCFS, including PEG, extended analogs, TCFS-PLA and TCFS-PLGA systems, Vitamin-E derived hybrids, and the next-generation stimuli-responsive architectures. The review focuses on the structure-function relationships by the variation of PEG chain length, ester linkage, and hydrophobic block composition that determine not only critical micelle concentration, colloidal stability, and intracellular trafficking, but also efflux modulation. Conjugation with TCFS does not always lead to performance improvements; in fact, optimally engineered polymer conjugates may enhance micellar stability and intracellular drug retention, but at the same time excessive PEGylation or overly stable linkers may weaken membrane interactions and thus decrease P-gp inhibition. Various translational challenges, such as formulation instability, dose-dependent toxicity, scalability, and regulatory considerations, are pinpointed and discussed at length to put the success accomplished in the laboratory in front of the clinical feasibility. Novel uses of TCFS conjugates in gene delivery, mRNA carriers, and combination nanotherapeutics are however, appraised with respect to these limitations. A serious framework for the balanced development of following generation nanocarrier systems is provided by this review. Thus, integration of polymer chemistry with biological response and translational considerations can promote TCFS polymer/prodrug conjugates as a tunable yet design-sensitive platform. Tocofersolan’s (TCFS) biophysical performance is determined by structural changes The kinetics of drug release and micellization are influenced by PEG length and linker chemistry Variations of hydrophobic tocopherol alter P-gp inhibition and membrane interaction Comparative insights relate pharmacokinetics and bioavailability to the design of TCFS analogs A platform for tunable excipients in next-generation nanotherapeutics is TCFS
