Key result
ECG criteria show low sensitivity for LVH, while inflammatory markers track with LVH severity.
Why the study?
The study aimed to evaluate the diagnostic performance of ECG criteria and inflammatory markers (IL-6, TNF-α, and CRP) in detecting left ventricular hypertrophy using echocardiography as the gold standard.
Do electrocardiographic criteria (Sokolow-Lyon, Cornell voltage, Peguero-Lo Presti) and serum inflammatory markers (IL-6, TNF-α, CRP) accurately diagnose and stratify left ventricular hypertrophy compared to echocardiography in adults?
Cross-Sectional (n=291)
No
Do electrocardiographic criteria (Sokolow-Lyon, Cornell voltage, Peguero-Lo Presti) and serum inflammatory markers (IL-6, TNF-α, CRP) accurately diagnose and stratify left ventricular hypertrophy compared to echocardiography in adults?
Standard ECG criteria have very low sensitivity (10.37%–20.75%) despite high specificity for detecting concentric LVH, whereas serum inflammatory markers (IL-6, TNF-α, and CRP) strongly correlate with LVH severity and may aid in diagnostic assessment.
ECG criteria lack sensitivity for LVH diagnosis; leaves open whether inflammatory markers aid severity stratification in prospective studies.
OBJECTIVE: Aim: To evaluate the diagnostic performance of ECG criteria and inflammatory markers-interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and C-reactive protein (CRP)-in detecting LVH. PATIENTS AND METHODS: Materials and Methods: A total of 241 patients with concentric LVH confirmed by ECHO were included and categorised into ECG-positive and ECG-negative groups. Additionally, 50 age-matched controls without LVH were recruited. The diagnostic performance of Sokolow-Lyon, Cornell voltage, and Peguero-Lo Presti ECG criteria was assessed using sensitivity, specificity, and accuracy. Serum levels of IL-6, TNF-α, and CRP were analysed across different levels of left ventricular mass index (LVMI). RESULTS: Results: Significant differences were observed between ECG groups in interventricular septal thickness, left ventricular diameter, and LVMI (p < 0.05). ECG criteria demonstrated low sensitivity (10.37%-20.75%) but high specificity (up to 100%), with limited overall accuracy. In contrast, IL-6, TNF-α, and CRP levels were significantly elevated in patients with severe LVH (p < 0.001). CONCLUSION: Conclusions: ECG criteria show high specificity but limited sensitivity in diagnosing LVH. Elevated inflammatory markers correlate strongly with disease severity, suggesting their potential role in improving diagnostic accuracy.

Alhibaly et al. (2026) conducted a cross-sectional in Concentric left ventricular hypertrophy (LVH) (n=291). ECG voltage criteria (Sokolow-Lyon, Cornell, Peguero-Lo Presti) and serum inflammatory markers (IL-6, TNF-α, CRP) vs. Echocardiography as gold standard and non-LVH control group was evaluated on Diagnostic performance of ECG criteria for concentric LVH and serum levels of IL-6, TNF-α, and CRP across LVMI categories (p=<0.001).
