Novel RECD tick biomarker identifies pre-VF patterns across archival Holter recordings without reported clinical outcomes.
Why the study?
Wearable medical devices and ICU monitors increasingly rely on supervised labels to train alarms, but labels are scarce, delayed, and often unavailable when bedside systems must detect reorganisation among biosignal modules. This work outlines a translational and cautionary framework for integrating the regulated RECD tick as a label-free algorithmic biomarker candidate under pre-registered protocols.
This methodological and translational preprint outlines design constraints and a pre-registration template for integrating the label-free RECD tick ordinal biosignal biomarker into wearable and ICU monitoring systems without making clinical or regulatory claims.

No support for clinical use of RECD tick; leaves open its role as a label-free biomarker pending prospective validation.
Wearable medical devices and intensive-care monitors increasingly rely on supervised labels to train alarms. Labels are scarce, delayed, and often unavailable when a bedside system must decide whether relational structure among biosignal modules has already reorganised. Physics-Informed Ordinal AI (PIO-AI) and the Systemic Tau stack supply ordinal concordance, absolute concordance change with cancellation index κ, beyond-pairwise residuals (Res_pair / excess³), and the Discrete Extramental Clock (RECD) whose laboratory tick takes the gated form ΔT = g(τ_s)·|τ_s|. An archived Holter deposit examined sign concordance of the Lean RECD tick contrast and Level-3 excess³ on ten pre-ventricular-fibrillation (pre-VF) Sudden Death Database recordings under a stated protocol (DOI 10.5281/zenodo.23383640). That deposit is exploratory and archival: it reports protocol-bound concordance counts, not a cleared diagnostic device, wearable product claim, or ICU performance. This preprint treats successful extension of such concordance studies under pre-registered protocols as a research door—not a regulatory shortcut—toward integrating the regulated RECD tick as a label-free algorithmic biomarker candidate in wearable and ICU design. The contribution is translational and cautionary. Structure estimators are separated from alarms and from clinical decisions; wearable/ICU design constraints and a pre-registration template are stated before any bedside claim. Parallel ordinal-control translation in financial AI (DOI 10.5281/zenodo.23636566) is cited only as an architectural cousin. No new patient outcomes, sensitivity/specificity figures, or device clearances are invented or implied. Johel Padilla (2026) studied Pre-ventricular fibrillation (n=10). Discrete Extramental Clock (RECD) tick biomarker was evaluated on Sign concordance of the Lean RECD tick contrast and Level-3 excess³. The regulated RECD tick is proposed as a label-free biomarker candidate based on 10 archival pre-VF Holter recordings, with no new patient outcomes reported.
