This community service activity implemented SIKEMAS (Sistem Informasi Pencatatan Kesehatan Masyarakat), a web-based health information system, to support Posyandu service recording in Selur Village, Ponorogo, where paper registers were previously used. The activity was conducted through four stages: field observation, system development, implementation accompanied by training for community service students and village cadres, and evaluation. During a Posyandu session on 14 August 2026, three users—two community service students from a health study program assisting village cadres and one administrator—recorded health examinations of 37 toddlers. Nutritional status was analyzed for 34 toddlers using z-scores based on the Indonesian Child Anthropometry Standards (Ministry of Health Regulation No. 2 of 2020). Centralized data storage enabled recap and analysis without manual re-entry. SIKEMAS has the potential to minimize data loss and duplicate records, although its quantitative impact was not measured. Anthropometric analysis identified 5 toddlers (14.7%) as underweight or severely underweight and 7 toddlers (20.6%) as stunted or severely stunted; these figures reflect a single-session recording rather than village-wide prevalence. Continuous cadre assistance and data validation are required to improve record completeness in subsequent Posyandu activities.
