Preclinical systematic review evaluates small-molecule PCSK9 modulators on lipid metabolism.
Why the study?
The available preclinical evidence on synthetic and naturally derived small-molecule modulators of PCSK9 needed to be identified, characterized, and synthesized to map the preclinical landscape and highlight evidence gaps.
Do synthetic and naturally derived small-molecule PCSK9 modulators reduce total cholesterol and modulate PCSK9/lipid metabolism in preclinical cell-based and non-human animal models?
Meta-Analysis
Do synthetic and naturally derived small-molecule PCSK9 modulators reduce total cholesterol and modulate PCSK9/lipid metabolism in preclinical cell-based and non-human animal models?
This systematic review and meta-analysis synthesizes preclinical evidence on small-molecule PCSK9 modulators and their effects on total cholesterol and lipid metabolism to guide future translational research.

Small-molecule PCSK9 inhibitors need human trials before any clinical consideration; leaves open translational potential from preclinical models.
This systematic review aims to identify, characterize, and synthesize the available preclinical evidence on small-molecule modulators of PCSK9. The review focuses primarily on experimental studies conducted in cell-based/in vitro systems and non-human animal models. It evaluates synthetic and naturally derived small molecules capable of modulating PCSK9 expression, activity, secretion, degradation, or its interaction with the low-density lipoprotein receptor (LDLR). The review characterizes the compounds investigated, experimental models, mechanisms of action, doses or concentrations, and reported effects related to PCSK9 and lipid metabolism. A narrative synthesis is used to summarize the available preclinical evidence. Where sufficiently comparable quantitative data are available, particularly for total cholesterol, a meta-analysis is conducted to estimate the magnitude of treatment effects. The review is intended to provide an overview of the current preclinical landscape of small-molecule PCSK9 modulation, identify promising pharmacological strategies, and highlight gaps in the available evidence that may guide future experimental and translational research.

Arrieta et al. (2026) conducted a meta-analysis in Dyslipidemia / Lipid metabolism (preclinical models). Small-molecule PCSK9 inhibitors/modulators (synthetic and naturally derived) was evaluated on PCSK9 modulation (expression, activity, secretion, degradation, or LDLR interaction) and lipid metabolism effects (particularly total cholesterol). Small-molecule PCSK9 modulators were evaluated in a preclinical systematic review and meta-analysis of in vitro and animal models, with no quantitative results reported in the abstract.
