Key result
Pre-ablation serum biomarkers showed a non-significant pooled association with atrial fibrillation recurrence after catheter ablation (HR 1.33, 95% CI 0.99-1.80).
Why the study?
Atrial fibrillation recurrence after catheter ablation may be predicted by pre-ablation serum biomarkers, highlighting the need to consolidate evidence to improve patient selection.
Do pre-ablation serum biomarkers, clinical factors, ablation strategies, and predictive models predict or reduce atrial fibrillation recurrence in adults undergoing catheter ablation?
Meta-Analysis
Do pre-ablation serum biomarkers, clinical factors, ablation strategies, and predictive models predict or reduce atrial fibrillation recurrence in adults undergoing catheter ablation?
Pre-ablation serum biomarkers, particularly inflammatory markers like NLR and composite scores like the ABCD score, along with AI risk models, show potential to improve risk stratification for atrial fibrillation recurrence after catheter ablation, though pooled biomarker estimates were not statistically significant (HR 1.33, 95% CI 0.99–1.80) and substantial heterogeneity remains.
May refine AF ablation candidate selection; extends pooled biomarker associations but needs prospective validation.
Atrial fibrillation is a common arrhythmia whose recurrence after ablation may be predicted by serum biomarkers, highlighting the need to consolidate evidence to improve patient selection. The objective was to identify pre-ablation serum biomarkers associated with atrial fibrillation recurrence after catheter ablation. A systematic review and meta-analysis were conducted according to the PRISMA guidelines, with searches performed in PubMed, Embase, and Scopus using the descriptors “Catheter Ablation,” “Biomarkers,” and “Atrial Fibrillation,” combined with the Boolean operators AND/OR. Thirty studies were included, most of which demonstrated good methodological quality. Inflammatory biomarkers and composite scores showed promising results, whereas clinical factors, ablation strategies, pharmacological interventions, technical aspects, and risk and artificial intelligence models showed heterogeneous findings. The findings indicated the potential of biomarkers and predictive tools to improve recurrence risk stratification. However, pre-ablation serum biomarkers, combined with clinical factors and predictive models, may assist in patient selection and individualized management for ablation, although the heterogeneity of the evidence requires validation in multicenter studies.

Carvalho et al. (2026) conducted a meta-analysis in Atrial fibrillation undergoing catheter ablation. Pre-ablation serum biomarkers vs. Normal or reference biomarker levels was evaluated on Atrial fibrillation recurrence after catheter ablation (HR 1.33, 95% CI 0.99-1.80). Pre-ablation serum biomarkers showed a non-significant pooled association with atrial fibrillation recurrence after catheter ablation (HR 1.33, 95% CI 0.99-1.80).
