MICS via right midaxillary thoracotomy achieves unobstructed flow at 6 months in supracardiac TAPVC.
Why the study?
Minimal invasive cardiac surgery for supracardiac TAPVC repair via vertical midaxillary right thoracotomy had not previously been reported in Bangladesh.
Case Report (n=1)
No
This first reported case from Bangladesh demonstrates the feasibility of minimally invasive supracardiac TAPVC repair via vertical midaxillary right thoracotomy in a pediatric patient, resulting in unobstructed pulmonary venous flow at 6 months and early postoperative recovery.
May support MICS via VMRT in select pediatric TAPVC cases; leaves open prospective validation before wider adoption.
A 3 years 5months old girl body weight 11.2 kg, diagnosed case of supracardiac TAPVC (Total anomalous pulmonary venous connection) with large Atrial septal defect (ASD) admitted for surgical repair. MICS (minimal invasive cardiac surgery) via Vertical Midaxillary Right Thoracotomy(VMRT) done on 20th march 2025 and discharge on 5th post operative period with stable hemodynamics. At six month follow up Echocardiogram and check CT angiography showed unobstructed pulmonary veinous flow to left atrium. This procedure provides good early recovery and highly parent satisfaction.

Eliyas Patwary (2026) conducted a case report in Supracardiac total anomalous pulmonary venous connection (TAPVC) with large atrial septal defect (ASD) (n=1). Minimal invasive cardiac surgery (MICS) via vertical midaxillary right thoracotomy (VMRT) was evaluated on Unobstructed pulmonary venous flow to the left atrium on echocardiogram and CT angiography. MICS via vertical midaxillary right thoracotomy for supracardiac TAPVC enabled discharge on postoperative day 5 and unobstructed pulmonary venous flow at 6 months.
